One of the biggest challenges in coding knees occurs with the determination of reporting CPT 29877, arthroscopy knee, surgical; debridement/shaving of articular cartilage (chondroplasty) vs. CPT 29879, arthroscopy knee, surgical; abrasion arthroplasty (includes chondroplasty where necessary) or multiple drilling or microfracture.
- Reporting 29879 incorrectly when performed in separate compartment as opposed to 29877 x 1 regardless of the number of compartments performed. As indicated in the preceding description above, the correct CPT code selection is 29879; 29879-59.
- Incorrectly reporting CPT 29877 vs. HCPCS Level II Code G0289 when indicated. - Arthroscopic knee surgery. CPT code 29879 includes resection of osteophytes when performed in the same compartment. These should not be coded out separately.
- Colonoscopy. When a surgeon dictates that a polyp was biopsied and cauterized at the same time, use CPT 45384 (colonoscopy with removal of tumor(s), polyp(s), or other lesion(s) by hot biopsy or bipolar cautery).
- Hysteroscopy with polypectomy versus resection of a leiomyomata. CPT 58558 is a hysteroscopy including a polypectomy. However, the removal of a leiomyomata (CPT 58561) is a separately billable procedure with no bundling issues.
- Platelet-rich plasma injection. New code CPT 0232T (injection(s) platelet-rich plasma, any tissue, including image guidance, harvesting and preparation) went into effect July 1.
- Double osteotomy procedures. When a combination of osteotomy procedures is performed on the phalanx and metatarsal of the same toe, this would be considered a double osteotomy and should be coded with the CPT 28299 code rather than being coded separately.
